Assisted Living Social Worker / LMSW
We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our team as an Assisted Living Social Worker. In this role, you will be responsible for providing social work services to residents in our assisted living facility, assisting them in navigating the challenges of aging and helping them maintain their independence and quality of life.
Key Responsibilities:
- Conduct initial assessments and ongoing evaluations of residents to determine their social, emotional, and psychological needs
- Develop and implement individualized care plans to address residents’ needs and goals
- Provide emotional support and counseling to residents and their families
- Advocate on behalf of residents to ensure they receive appropriate care and services
- Collaborate with other healthcare professionals, including nurses, physicians, and therapists, to provide comprehensive care to residents
- Facilitate group activities and support groups to promote social engagement and well-being
- Provide education and resources on community services, benefits, and legal issues related to aging
- Document all interactions with residents and maintain accurate and up-to-date records
Qualifications:
- Master’s degree in Social Work from an accredited program
- Current licensure as a Licensed Master Social Worker (LMSW)
- Minimum of 2 years of experience in social work, preferably in a healthcare or assisted living setting
- Knowledge of aging-related issues and resources
- Strong communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
- Compassionate and patient approach to working with older adults and their families
